NO WARRANTY OR GUARANTEE is given or implied by the Optimum Wellness
Naturopathic Medical Clinic, or by members of its staff, that the testing or the
treatment received at the clinic will effect a cure of symptoms or complaints
for which the patient seeks treatment.
Patients by presenting themselves with the request for Prolotherapy,
acknowledge that they have not been induced or solicited to take treatment by
the doctors or any staff member. The treatment of Prolotherapy is given
only at the patient's request, with their written and implied agreement and with
their full knowledge of the limitations and potential complications of
treatment.
Prolotherapy is given solely for the purpose of treating chronic pain or as a
nonsurgical ligament reconstruction.
